Select this option if you would like to become a team
captain of a group of 10 or more participants. From here you will be able to create a team directly in Active.com and
once complete your team name will then be available in real time for you and your memebers to join

Select this option if you would like to register online
to walk with a pre-registered team (and pay by credit card only) . Please note that you must know the team
name you would like to join before you can register. Once you know your team name & captain you will be directed to the
Active.com registration page and you will have the ability to register yourself and up to 5 more members of the same household.

Select this option if you would like to register for
the event online as a non team participant (and pay by credit card only). During the registration process you will be
able to register up to 6 family members in one household. If you decide to join or form a team at a later date, just
email us at helpdesk@butterflywalk.com and we can make this change for you.

Select this option if you would like to register online
for this event and mail in a check. Selecting this option will take you to an online form through our website (NOT
Active) and here you will have the ability to register yourself and up to 5 more members of the same household as a team or non team participant.
If you are registering on a team, please note that you must know the team name you would like to join before
you can register.

this option if you would like register or donate by mail instead of online. Simply fill out the necessary information
on the registration form or pledge coupon and mail it in along with your payment. Your forms need to be received by
April 22th, 2011.
