Captain's Pledge

To become a team captain:

To become a team captain, you need fill out the following information and agree to the pledge below.  Once you have submitted this form you will be directed to our Active registration page and have the opportunity to register yourself and up to 5 more members of your household.  Due to liability/waiver release, you will only be able to register family members in your household and not additional team members.  Your team members will need to register themselves once your team has been established.

By clicking submit you are agreeing to the following responsibilities:
  • I will set a goal to have ten or more people join my team & have them register themselves/family for the Butterfly Walk.
  • I have determined a fundraising goal over $500 for my team to reach, and will encourage team members to seek donations to help achieve that goal.
  • I understand that as a team captain I have the ability to establish a team fundraising page on ActiveGiving.com. I will create this page and encourage team members to use this team page to solicit donations.
  • I will (or will make arrangements to) pick up our team's T-shirts and wristbands at a designated location a day or two prior to the event and will plan on bringing any offline donations received at that time.
